What is the difference between a panel clinic/hospital and a non-panel clinic/hospital?
Ulink Myanmar will not issue a LOG to a clinic/hospital that is not in the panel. If members prefer to visit a non-panel clinic/hospital, please pay the clinic’s invoice before submitting a claim for the medical charges through email. Ulink shall process a reimbursement for coverable charges under members’ insurance policy.

Ulink will verify members’ details and email the LOG to the clinic and members upon confirmation. Member must bring along LOG (either softcopy or hard copy), membership card, and photo ID/passport.
How to request an LOG for inpatient admission/ surgery?
For inpatient admission/surgery, please notify Ulink at least 3 to 5 working days before or, for emergency cases, as early as possible to avoid delay in LOG issuance. For treatments estimated to cost above USD 1,000 or for treatment outside Myanmar, please inform Ulink within working days (10) days as possible as further approval from members’ insurer is required and the time required for approval depends on a case-by-case basis depending on the complexity of the case.
Members are to arrange admission and email required info to Ulink at the email address stated on members’ insurance card. The required information includes
members’ name,
NIRC no. or date of birth,
policy number,
company name,
admission date,
hospital name,
treating doctor name,
length of hospital stays and
all medical reports related to the admission with estimated cost (if any) from the hospital.
Ulink will verify members’ details and medical reports and issue an initial LOG to the clinic/hospital and members upon confirmation. Members must bring along LOG (either softcopy or hard copy), membership card and photo ID/passport.
In the case of an extension of hospitalization stay or if a superseded LOG is required, a member or hospital must inform Ulink before the extension period.
How do I check if my treatment is coverable?
Name, NRIC no., policy number, the purpose of the visit, Medical records what surgery or procedure is being planned (if applicable).
Members’ agent or Ulink Myanmar will get back within 48 hours with more information on whether members’ treatment is coverable under current policy.

How long is the LOG valid for?
What are important things to note and T&C for use of LOG service at panel hospital/ clinic?
Failure to provide or provision of incomplete medical information and documents may result in the delayed issuance of LOG as well as a rescheduling of elective admission.
Authorization and/or issuance of LOG does not guarantee benefits or coverability, and all claims shall be assessed by the relevant claim department for their validity.
Ulink Assist Co Ltd reserves the right to reject or withdraw any LOG if the final diagnosis is related to a policy exclusion.
Hospitals have the discretion to request additional payments as required by their internal practices even with the presentation of the LOG.
The insured member or policy owner agrees to pay any excess amount and uncovered medical expenses (if any) to clinics/hospitals or Ulink Assist Co Ltd within 7 business days from the date of request. The actual covered amount is subject to the information provided in the pre-authorization form, actual diagnosis, eligible policy benefit, exclusion clause, terms, and conditions stated in the policy document.
After hours and public holidays – In the case where the member does not have preauthorized LOG and the hospital cannot accept immediate LOG at that time, the member has the option to do reimbursement.
After seeking treatment at a hospital, how do I submit a claim?
receipts/invoices, including name & date
medical certificates,
lab test results, and
all other medical reports (issued in English by the attending doctor/hospital)
For hand-written reports, please kindly provide Dr’s SAMA, Signature/Stamp
to Ulink Assist within 60 days of the insured event, otherwise, claims may be declined.
The invoices should include a summary and a detailed breakdown of fees.
Reimbursement of eligible claims will be made directly to the insured member’s company account (KBZ / CB ) by wire transfer/cheque.
For claim submission, please send via email with the email subject following format:
“Name _ Policy number/Company Name _ Date” (E.g. “AyeAye_00000TML00/xxx Company_10Dec2018”).
