How long need to wait LOG service?

If time to appointment is after 24 hours: Next calendar day
If time to appointment is within 24 hours: Members can expect to receive the LOG within a day
Non-Emergencies: LOG issued within 6 hours
Emergencies: LOG issued within 3 hours

How long is an LOG valid for?

LOG is valid for 1 week from the date of issue. For any change of diagnosis, treating doctor or appointment needs to be rescheduled, please email Ulink to issue a revised LOG.

How to request LOG for In Patient hospitalization?

Applying a Letter of Guarantee before hospitalization:

Member need to send required info for verification to healthclaim@daiichilife.com.mm  and after that member will receive Member Declaration Form and Pre-Admission Form (can be downloaded from www.ulinkmyanmar.com/daiichi)

What is the required information for verification and process?

Please email/call Ulink and provide the below information for verification:

  1. Full Name as per NRC
  2. Insured client ID / NRC
  3. Date of Birth
  4. Phone Number, Email Address
  5. Appointment/Admission date
  6. Hospital/ Clinic Name
  7. Treating doctor Name
  8. Diagnosis or Brief medical information including the duration of symptoms
  9. IPD/OPD
  10. Estimated cost
  11. Pre-admission form (Completed and signed) (by Insured and Attending doctor)
  12. Member declaration form (Completed and signed) (by Insured)
How to request Cashless LOG service for Emergency Accident Treatment?

For Cashless Request related to Emergency Accident Treatment, some of the following information will be required for the further assessment.

  1. Date and time of the accident
  2. Type of the accident
  3. Whether the insured member is rider or passenger (for motor vehicle accident)
  4. Is the insured member drink alcohol during the accident
  5. Is the insured member wear the helmet during the accident (for motor vehicle accident)
  6. Is the insured member used illegal drugs during the accident
  7. Is the accident related to assault
  8. Diagnosis detail (How do you feel?)
  9. Please describe the mechanism of accident.

Note: Ulink may contact member if additional information or document is required to assess eligibility and coverage.

after that member will receive a Letter of Guarantee (E-LOG) via email (if the request is eligible and approved)

After submitting the required information

Member must bring the LOG (either softcopy or hardcopy), photo ID/passport and membership card to the clinic/hospital.

For in-patient cases, Ulink will issue an Initial LOG and the superseded LOG will be provided as required after the daily follow-up with the related providers.

At the time of discharge, Ulink to check the final bill for uncovered items, deductible and co-insurance amount and the final LOG will be issued. The Hospital will collect the shortfall amount from the Insured member.

After seeking treatment at a hospital, how do I submit a claim?

To submit a claim,

  • Download claim form from Ulink Assist website
  • Fill out the claim form and email to Ulink (within the required 90 days)
  • Attach all the supporting documents together with complete claim form
    1. Complete medical claim form
    2. Medical records, Doctor recommendation letter
    3. Investigation results
    4. Invoices with detailed breakdown
How do I check my status of the claim?

Please email to the email address healthclaim@daiichilife.com.mm with Name, Insured Client ID, Ticket Number and contact information.
