Things to know before claim submission
Step 1. Make sure to attach the required supporting documents
Step 2. Submit Claim online for Death benefit (below)
Step 3. Claims assessed by health claim team
Step 4. Get your claim approval within 14 days
Step 5. Receive reimbursement via bank transfer


Required Information and Documents
- Member information (Name, DOB, Policy Number, etc.)
- Bank information
- Duly completed claim form
- True copy of Death Certificate or Conclusive Evidence
- Consent Form and Attending physician’s statement if any medical record from hospitals and clinics accredited by the Ministry of Health (ICU record, if admitted to an ICU)
- Copy of the documents such as ID Card, Household List, Birth certificate, Marriage certificate or other relevant documents
- Police report and medical report, if relevant
- Any other documents or information required by Dai-ichi Life
