How to request a LOG for Inpatient admission/ surgery?
For Inpatient admission/surgery, please notify Ulink at least 3 to 5 working days prior to the admission/surgery or, for emergency cases, as early as possible to avoid delay in LOG issuance as Ulink needs further approval from the Insurer. The duration for LOG approval will depend on case-by-case basis accordingly.
Ulink will verify members’ details and medical reports and issue an initial LOG to the clinic/hospital and members upon confirmation. Members must bring along LOG, NRC card with photo ID (either softcopy or hard copy).
In the case of an extension of hospitalization stay, a member or hospital must inform Ulink before the extension period.
How to request a LOG for emergency admission?
For emergency Inpatient cases, please visit the nearest panel hospital/clinic and contact to Ulink. Ulink will issue a LOG to the hospital/clinic for coverable charges under members’ insurance policy after verification and assessment. Members can then proceed for treatment and the clinic/hospital will send an invoice for members’ eligible medical charges to Ulink Myanmar.
How long is the LOG valid for?
LOG is valid for 1 week from the date of issuance. For any change of diagnosis, treating doctor, or appointment time, please email Ulink to issue a revised LOG.
Who pays for expenses not covered by compensation?
Must be paid by the insured himself.
What is the waiting period for maternity care?
The maximum benefit of the policy for maternity care coverage will be paid only if the policyholder’s estimated date of birth is after a waiting period of at least 10 months from the date of initial coverage of the policy.
