Required Information and Documents
- Member information (Name, DOB, Policy Number, etc.)
- Bank information (Need Delegation letter)
- Brief medical information (Diagnosis, Symptoms start date, treatment date, Hospital name, etc.)
- Attach medical records, operation report, investigation report, discharge summary etc.
- Attach invoices with detail breakdowns
