Things to know before LOG Submission
- Member makes appointment at Ulink’s Panel Hospitals and requests LOG
- Member provides the required member information and brief medical condition
- Ulink verifies the member
- If member is verified, member submits pre-admission form, member declaration form and medical records upon request
- Ulink approves the coverage and issues LOG for eligible cases
- Member receives treatment cash-lessly at hospitals


Required Information and Documents
- Member information (Name, DOB, Policy Number, etc.)
- Hospital admission date
- Hospital Name
- Doctor Name
- Length of stay (optional)
- Estimated amount (optional)
- Brief medical information (Diagnosis, Symptoms start date, treatment date, Hospital name, etc.)
- Attach medical records, investigation results, and Doctor recommendation letter
