Things to know before LOG Submission

  1. Member makes appointment at Ulink’s Panel Hospitals and requests LOG
  2. Member provides the required member information and brief medical condition
  3. Ulink verifies the member
  4. If member is verified, member submits pre-admission form, member declaration form and medical records upon request
  5. Ulink approves the coverage and issues LOG for eligible cases
  6. Member receives treatment cash-lessly at hospitals

Required Information and Documents

  1. Member information (Name, DOB, Policy Number, etc.)
  2. Hospital admission date
  3. Hospital Name
  4. Doctor Name
  5. Length of stay (optional)
  6. Estimated amount (optional)
  7. Brief medical information (Diagnosis, Symptoms start date, treatment date, Hospital name, etc.)
  8. Attach medical records, investigation results, and Doctor recommendation letter
