What is the difference between a panel clinic/hospital and a non-panel clinic/hospital?
Ulink Myanmar can issue a Letter of Guarantee (LOG) for treatment at a panel clinic/hospital. With a LOG, member does not need to pay the clinic/hospital if member’s treatment is coverable under member’s insurance policy. Instead, the clinic/hospital will invoice Ulink directly for member’s medical charges. However, the clinic/hospital will collect from the member if there is an excess, or uncoverable items.
For non-panel hospital/clinic visit, please submit claim directly to AMI.
Where can I find a list of Ulink Myanmar’s panel clinics/ hospitals?
I want to get Cashless service for my treatment/ hospital admission.
Member can request the cashless LOG via email and after verification and assessment, we will send the Member Declaration Form and Pre-admission Form to the Member’s E-mail through healthclaims@amilifeassurance.com within (2-3) hours in order to be able to use the Cashless service (Or) member can be downloaded the Form on the www.ulinkmyanmar.com/amilife website.
After filling out the form completely, send it to healthclaims@amilifeassurance.com
As a member, you must bring ELOG and NRC.

How to request LOG for Outpatient GP & specialist medical appointment?
For Outpatient appointment for weekdays within office hours, please send LOG request to Ulink with full information for LOG request. For Outpatient GP and specialist appointment after office hours and public holidays, please do pay and claim directly to AMI.
How to request a LOG for Inpatient admission/ surgery?
For Inpatient admission/surgery, please notify Ulink at least 3 to 5 working days prior to the admission/surgery or, for emergency cases, as early as possible to avoid delay in LOG issuance as Ulink needs further approval from the Insurer. The duration for LOG approval will depend on case-by-case basis accordingly.
Ulink will verify members’ details and medical reports and issue an initial LOG to the clinic/hospital and members upon confirmation. Members must bring along LOG, NRC card with photo ID (either softcopy or hard copy).
In the case of an extension of hospitalization stay, a member or hospital must inform Ulink before the extension period.
How to request a LOG for emergency admission?
For emergency Inpatient cases, please visit the nearest panel hospital/clinic and contact to Ulink. Ulink will issue a LOG to the hospital/clinic for coverable charges under members’ insurance policy after verification and approval. Members can then proceed for treatment and the clinic/hospital will send an invoice for members’ eligible medical charges to Ulink Myanmar.
How long is the LOG valid for?
LOG is valid for 1 week from the date of issuance. For any change of diagnosis, treating doctor, or appointment time, please email Ulink to issue a revised LOG.

Can I claim for pregnancy/child birth?
In case of Miscarriage, if you have Optional (1) benefit plan, it can be covered as per policy plan.
